Pre-Visit Instructions

 WHAT TO BRING TO YOUR FIRST VISIT

  • Identification
  • Your insurance card(s)
  • Any referrals (if required by your insurance company)
  • Any x-rays that our physicians have ordered for you
  • A list of medications that you are currently taking (form included in printable registration forms below) 
  • Any co-payments that are required at the time of the visit (if applicable)
  • Patient Forms - please print forms, complete and bring to your appointment. Use the link below to download the forms needed.   Please bring the completed forms with you to your appointment.  If you are unable to print these, we will have forms available in our office and please allow time to complete these prior to appointment. 

Patient Registration Form  - Print this form and bring completed to your appointment and be entered into a drawing for using our 'newly improved' website.  Having this filled out in advance will also save time in checking in for your appointment.

Payment and Insurance

Urology Associates of Wisconsin, S.C., is a provider for Medicare and most major insurance plans. We provide insurance billing.  Please contact your insurance company to find out if we are part of your benefit plan network.  Insurance companies offer many different networks of providers and the only way to know for sure that any provider is part of your network is to get that information directly from your insurance company.  If there are any issues with your claims and you called your insurance company to verify this prior to an appointment, then your insurance company should have that call documented.

We request payment at the time of your appointment for services that are not covered by insurance. We accept MasterCard and Visa. When necessary, our staff will work closely with patients who require a payment plan.

If you have any questions regarding which insurance plans we typically accept or any patient billing concerns, please contact us. Questions regarding your coverage and benefits should be directed to your employer or insurance company.  .
