Making a Credit Card Payment
For your convenience we have included our credit card payment authorization form if you should need this to make a payment. Please click on link to download this form. You can either bring to our office or mail it to us at the Neenah office. You may also call in any credit card payments.
Financial Assistance Application
Urology Associates wants to be understanding of our patients' ability to pay their obligations that presents the least amount of difficulty to the patient involved, but satifies the need for out practice to meet our financial responsibilities to our employees, state regulations and creditors. If you are having trouble paying your bills or cannot meet the required monthly payment amount that has been determined, by our billing office, please complete our Financial Assistance Application. Applications are reviewed by the Financial Assistance Team too see if you qualify for lower monthly payments or financial assistance from our clinic.
